INSTRUCTIONS FOR SUBMITTING A NOMINATION
NOMINATIONS NOW CLOSED!
Nominations must be sent in print to the GTHA office by mail or courier.
A nomination form must accompany each entry. Please submit FIVE (5) copies of your completed nomination forms (include all pages) and supporting materials. Late Nominations will NOT be accepted.

Once nominations are received a package with personalized invitations for each nominee submitted will be sent to the General Manager by courier.
TIP: The Judging Committee recommends using all the allowable space when submitting nominations. Answer all questions and get creative! Include stories, pictures or any other material.
REMINDER: Nominations will not be accepted if it exceeds allowable space in the forms. Any attached Materials and/ or samples submitted with your nomination will NOT be returned.

Don't forget to submit two (2) photos of your nominees' once your application has been submitted! See more details on how to submit photos here.

- I have completed the official GTHA nomination form.
- I have attached 5 copies of the complete nomination form and all sample materials.
- I have submitted 2 photos of each of the nominee.
